Medical history in one place What to gather and how to keep it: allergies and medication, reports and scans, operations and vaccinations, doctors and insurance, plus a one-page summary. [ ] Write a one-page summary Allergies, chronic conditions, regular medication, operations, doctor and family contacts. The single most valuable page. [ ] Record allergies and reactions separately and visibly To medication, food, materials. With a description of the reaction, not only the name — that affects decisions. [ ] List your regular medication Name, dose, timing, who prescribed it and when. Including supplements: they interact with treatment too. [ ] Collect reports, conclusions and results By date, oldest first. Even old findings show the trajectory and prevent unnecessary tests. [ ] Ask for imaging in digital form Discs stop being readable and printouts fade. Digital copies outlive both devices and house moves. [ ] Note operations and hospital stays What, when, where and any complications. It is the first question before any procedure or anaesthesia. [ ] Gather your vaccination records Dates and products. Needed for travel, admissions, work and changing clinics — and slow to reconstruct. [ ] Note the significant family history Conditions in close relatives that doctors ask about. Ask your parents while you can. [ ] List your doctors and clinics Who provides care, where the records are, how to book, where to go urgently. Contacts are needed before documents. [ ] Add your insurance details Numbers, validity, what is covered, the helpline. In the moment they are needed, these take longest to find. [ ] Digitise the papers and name files sensibly Date, what it is, who issued it. In three years \u201cscan_1.pdf\u201d helps nobody, including you. [ ] Agree who else has a copy Someone close should know where the summary and folder are. In an emergency, the person searching is not the one who filed them. [ ] Do the same for children and older relatives Their records are needed more often and usually remembered by one person. Changing doctors then saves weeks. [ ] Update after each appointment and once a year Five minutes after a visit instead of reconstructing three years from memory. Checklist from Unload: https://unloads.me/c/medical-records
